Your First Visit With Indiana Fertility Institute

Congratulations on taking the next step on your fertility journey. We’re excited to help you along your way. Here’s what to know about visiting us for the first time.

Everyone’s path to parenthood is unique. You’ll find that when you visit Indiana Fertility Institute, our approach is highly personalized to your specific situation. Our fertility specialists spend time getting to know each patient, including their health background and family-building goals. Whether you’re hoping to conceive right now or preserve your fertility for the future, we can create a treatment plan that’s right for you.

Before Your Appointment

Once you’ve scheduled an appointment with Indiana Fertility Institute, keep an eye on your email inbox. You’ll receive a link to register for our electronic medical records (EMR) system. Signing up involves setting a username and password. Please add all of the requested personal and medical details right away to secure your appointment. You can access our Patient Portal here.

We’ll spend most of your first visit going over your medical history, so it’s important that you fill out the forms included in your welcome packet, including the medical release form, as soon as possible. It's particularly important for us to be aware of any prior fertility testing, labwork, imaging, reports, etc.

Items You'll Need to Bring

Please arrive at our office or log onto the appointment link at least 15 minutes early on the day of your appointment. You’ll need to bring the following items with you:

  • Insurance card and/or information
  • A valid driver’s license or photo ID
  • Insurance co-payment or self-payment

What to Expect During the First Visit

Your provider will go over your medical records as well as your partner’s (if applicable) to garner a better understanding of your health background. Once we establish a general understanding of your current situation, we will recommend the appropriate diagnostic procedures to get to the root of why you’re having trouble conceiving. In many cases, we are able to schedule these tests and procedures right away. During this consultation, you and your partner will also have the chance to discuss any questions or concerns you may have about reproductive health, your medical history, and potential treatment options.

Following Your First Visit

With the initial consultation complete and a treatment recommendation in hand, you’ll have the opportunity to meet with one of our financial counselors. They can help you get a clear picture of the cost involved, what your insurance will cover, and options for payment. Everything is tailored to your unique situation.
