What are the signs of male infertility, and when should I get tested?

Male infertility is not something most men can detect on their own. Unlike many health conditions, it rarely produces noticeable physical symptoms. Many men have no reason to suspect an issue until they begin trying to conceive without success.

As a general guideline, a fertility evaluation is recommended after twelve months of unprotected intercourse without pregnancy, or after six months when the female partner is 35 or older. That said, certain circumstances may warrant earlier testing. Men with a history of reproductive surgery, testicular trauma, infections, cancer treatment, hormonal disorders, or known genetic conditions may benefit from an evaluation before attempting conception or sooner in the process.

Potential Causes of Male Infertility

Male infertility can arise from conditions affecting sperm production, quality, or delivery. Common causes and contributing factors include: 

  • Varicocele
  • Azoospermia (obstructive or non-obstructive)
  • Low sperm count (oligospermia)
  • Poor sperm motility or morphology
  • Hormonal imbalances (e.g., low testosterone)
  • Genetic or chromosomal conditions
  • Infections or structural blockages
  • Lifestyle or environmental factors
  • Unexplained infertility

Because fertility is influenced by multiple biological systems, diagnostic testing is often necessary to determine whether one or several factors are contributing to the problem.

What is a semen analysis, and why does it matter?

A semen analysis is a laboratory evaluation of a semen sample that examines several key indicators of sperm health. It is typically the first test recommended when evaluating male fertility because it is simple and non-invasive, yet can provide a significant amount of information about male reproductive health. 

During the analysis, a laboratory technician examines the sample for the following parameters:

  • Sperm count: the concentration of sperm present per milliliter of semen
  • Motility: the quality and direction of sperm movement
  • Morphology: the size and shape of sperm
  • Semen volume: the total amount of fluid ejaculate

Results are compared against established reference ranges to help your provider determine whether sperm health may be contributing to fertility challenges. 

Preparing for a semen analysis is straightforward. You will be asked to abstain from ejaculation for two to five days before your appointment. The sample is collected on-site in a private, comfortable setting, and results are generally available within a few days.

What other male fertility tests might be recommended?

A semen analysis can identify many male fertility concerns, but it does not always explain why those abnormalities are present. When additional information is needed, your physician may recommend further testing to evaluate hormonal function, genetics, or other factors that can affect fertility.  The specific tests used depend on individual circumstances, but may include any of the following:

Medical History and Physical Examination

A detailed medical history and physical examination lay the groundwork for a thorough evaluation. Your physician will ask about past surgeries, injuries, medications, lifestyle habits, prior infections, and relevant family history. A physical examination can also reveal findings that may impact fertility, including varicoceles or other structural abnormalities involving the testes and reproductive tract.  

Hormone Testing

Sperm production depends on proper hormone levels. A targeted blood panel measuring testosterone, FSH, LH, and prolactin gives your provider a window into whether fertility is being compromised by a hormonal imbalance. 

Genetic Testing

When sperm counts are severely diminished or absent, genetic factors may be playing a role. Chromosomal analysis, including karyotyping and Y-chromosome microdeletion testing, can identify hereditary conditions that affect the body's ability to produce sperm.

How much does male fertility testing cost?

The cost of a male fertility evaluation will depend on which tests are needed to reach a diagnosis, as well as insurance coverage. 

A semen analysis is generally among the more affordable components of an evaluation, while hormonal bloodwork, genetic testing, and other specialized diagnostics may involve additional considerations. Coverage for evaluations tends to vary by plan. While some policies provide at least partial coverage for diagnostic fertility testing, others may not offer any fertility-related benefits.

  • Many forms of male infertility can be treated successfully. Depending on the underlying diagnosis, treatment may involve medication, hormone management, surgery, lifestyle changes, or assisted reproductive technologies such as IVF. 

  • Although men continue producing sperm throughout adulthood, sperm quality can gradually decline with age. Changes in motility, morphology, and DNA integrity have all been associated with advancing paternal age. While many men conceive naturally later in life, fertility testing can provide a more accurate assessment of reproductive health. 

  • Abnormal sperm movement or shape can develop for many reasons. Common contributors include varicoceles, hormonal imbalances, infections, genetic conditions, tobacco use, excessive alcohol consumption, environmental exposures, and certain medications.

  • Yes. Certain medications and supplements can influence sperm production, hormone balance, and overall sperm quality. Testosterone replacement therapy, anabolic steroids, some antidepressants including SSRIs, chemotherapy medications, and certain blood pressure drugs are all examples of treatments that may affect fertility.

    Some supplements and antioxidants, including Zinc, CoQ10, and Vitamin C, may support sperm health when used appropriately, though excessive supplementation can sometimes have unintended effects. During your evaluation, your provider will review any medications and supplements you take regularly to determine whether they could be contributing to fertility concerns.

  • Typically, many causes of male infertility are temporary or improve with treatment. In situations where fertility is more significantly affected, your provider can discuss the full range of available options.
