Fresh vs. Frozen Embryo Transfer: How We Decide What's Right for You
Once your embryos have developed in the lab, one of the next big questions in an IVF cycle is when to transfer one: right away, in the same cycle as your egg retrieval, or later, after your embryos have been frozen. These two paths are called a fresh embryo transfer and a frozen embryo transfer (FET). A couple of decades ago, fresh was the default for almost everyone. Today, frozen transfers are extremely common — and for good reasons. Here's how the two approaches differ, what the research actually shows, and how your care team decides which one fits your situation.
What's the Difference?
The distinction comes down to timing.
In a fresh transfer, an embryo is transferred to the uterus a few days after your egg retrieval, within the same stimulated cycle. Everything happens in one continuous window.
In a frozen transfer, your embryos are frozen after retrieval and stored, and a single embryo is thawed and transferred in a later cycle — often the following month or beyond — after your body has had time to recover from the stimulation medications. The transfer itself feels the same to you; the difference is what happens with the embryos in between, and how your uterus is prepared.
Why Frozen Transfers Have Become So Common
Several developments have shifted many programs toward freezing:
Freezing became remarkably reliable. A flash-freezing technique called vitrification dramatically improved how well embryos survive the freeze-and-thaw process, to the point that a thawed embryo's potential is now very close to that of a never-frozen one. Genetic testing requires it. If you're doing preimplantation genetic testing (PGT-A), your embryos are biopsied and then frozen while the genetics lab completes testing — which takes time. It can lower the risk of OHSS. For patients who respond strongly to stimulation medication, transferring in the same cycle can raise the risk of ovarian hyperstimulation syndrome (OHSS). Freezing all the embryos and transferring later — sometimes called a "freeze-all" approach — sidesteps that risk.
It lets your body reset. Stimulation medications raise hormone levels well above what your body produces naturally. Some evidence suggests that transferring into a later, calmer cycle may offer a more natural, receptive uterine environment for implantation.
It adds flexibility. Freezing allows transfers to be scheduled around your life and health, supports fertility preservation, and gives your team time to optimize the timing of the transfer.
Does Fresh or Frozen Actually Work Better?
This is the question most patients want answered, and the honest response is: for many people, the odds of a live birth are similar either way. The early enthusiasm that "frozen is always better" has been tempered by more recent research showing comparable outcomes for a lot of patients.
Where a freeze-all approach does appear to help most is in patients who respond strongly to stimulation and produce many eggs — both by improving outcomes and by reducing OHSS risk. For patients who produce fewer eggs, the evidence for a freeze-all advantage is less clear, and a fresh transfer may be perfectly reasonable. In other words, the "better" option depends heavily on you — not on a blanket rule.
A Word on Pregnancy Outcomes
Beyond the chance of pregnancy, researchers have looked at how fresh and frozen transfers relate to pregnancy health, and the picture is nuanced. Studies suggest that fresh transfers are associated with a modestly higher chance of preterm birth and lower birth weight, while frozen transfers — particularly those using a medicated protocol — are associated with a somewhat higher chance of high blood pressure disorders in pregnancy (such as pre-eclampsia) and larger-than-average babies.
These are relative differences, not certainties, and both paths lead to healthy pregnancies every day. But they're one reason the type of frozen transfer protocol matters, and an area your physician is actively thinking about when planning your cycle. It's a good topic to raise directly with your care team.
How a Frozen Transfer Cycle Is Prepared
If you have a frozen transfer, your uterus needs to be readied so the timing lines up with your embryo's stage of development. There are a few common ways to do this:

Natural-cycle FET relies on your own ovulation to prepare the uterine lining, with minimal or no medication. There's growing interest in this approach, in part because keeping your body's natural hormone-producing structure (the corpus luteum) may be beneficial for pregnancy.

Modified natural-cycle FET uses your natural cycle with a little medication support and a trigger to time ovulation.

Programmed (medicated) FET uses estrogen and progesterone to prepare the lining on a controlled schedule, without relying on ovulation. This offers precise timing and flexibility.
Which protocol is right depends on your cycle regularity, medical history, and your team's judgment.
When Fresh Still Makes Sense
Frozen transfers are common, but fresh transfers absolutely still have a place. If you're not doing genetic testing, you're not at high risk for OHSS, and your uterine lining looks healthy in the retrieval cycle, a fresh transfer can be an excellent option — and it gets you to transfer sooner. The rise of frozen transfers isn't a sign that fresh is outdated; it's a sign that we now tailor the timing to each patient.
